Student
should deposit their pay-order/D.D/ Crossed
Cheque personally at college counter only. |

Candidate
should fill application form in his/her own
handwriting. |

All
the particulars in the application form must
be written carefully and legible handwriting.
Form which are incomplete, in any form, or
without sufficient particular and/or enclosure. |
|
The
application form duly filled in, should be
accompanied by the following documents the
originals of which shall have to be produced
whenever required: TC./ S.LC. of the school/
college last attended. |
|
An
attested true copy of matriculation/ Higher
Secondary certificate showing the birth of
the candidate. |
|
Student
passing examination other than the M.P. Board
should produce certificate from Rani Durgavati
University, Jabalpur (M.P.) and a migration
certificate of the previous Board/University. |
|
Foreign
student should submit a copy of student visa,
eligibility certificate from previous Board/
University. |
|
Three
recent passport size photographs. |
